Personally, I have started studying medicine in Cuba, but I have only finished the 3rd year of course because of moving to the US.

When patients visit a hospital, they meet a nurse and explain the problem. The nurse will relate the problem to previous one and the prescription that was used to the earlier patient. The nurse may decide to prescribe the medicine or treatment the doctor has to give to the patient.
